Salary, Bonus or Contract Query Form

You may be using a computer to submit a query regarding your salary, bonus or contract but the people reading your forms and processing your salary are human.  Therefore, we can make mistakes :-(  If we have got it wrong, please accept our apologies.  We're ready to respond and all we need you to do is give us the details regarding your query and we'll get straight onto it to put matters right.  Where there has been an under or overpayment, teachers are usually offered the choice to be paid or pay back straight away or for an adjustment to be made in the next salary pay day.  Once submitted, your form will be sent to members of the Executive.  Aside from the school accountant, we never discuss salaries, bonuses nor contracts with anyone outside of the executive.

Teachers are strongly advised to scrutinise their pay-slip the day they are paid.  Sifting through literally hundreds of paper files trying to identify a single hour of overtime a teacher thought they hadn't been paid for but subsequently had forgotten about calling in sick and missing an single class in the same month, a happy Linda Wu does not make.  It is also at Linda's discretion if she wants to investigate a query that goes back more than one calendar month.  Please be mindful of this.

A note on salary advances:
Teacher may apply for hardship assistance in extreme circumstances.  If a teacher finds themselves in a dire situation whereby money is needed before the next salary pay date, they may apply for up to 25% of their salary in advance.  Payment of hardship assistance is a discretionary payment, made in cash, where a minimum of 48 hours notice is given.  Funds should not be applied for or expected as a general course.
You may tick more than one option.
